N2  -  Summary. We report the case of a young, healthy and fit teenager who suffered ischemic stroke as the only symptom of covid-19 infection. A healthy, asymptomatic 18 y.o. athlete suffered ischemic stroke. Asymptomatic, unsuspected covid-19 infection was found upon admission. Stroke was caused by distal occlusion of middle cerebral artery, besides an asymptomatic thrombosis of a distal branch of the pulmonary artery was also found. No risk factors for stroke were found, besides a patent foramen ovale (PFO). Whatever the role that PFO might possibly have played in causing this stroke, it was clearly precipitated by a hypercoagulable state induced by ­covid-19 infection. While covid-19 infection in young people is often erroneously perceived as innocuous, it can have dire consequences even in young, healthy and fit subjects.
