N2  -  Summary. We report the clinical case of a former alcoholic treated with disulfiram who presented with acute-onset ataxic sensorimotor demyelinating polyradiculoneuritis, ophthalmoplegia, and altered mental status, with evidence of toxic-metabolic basal ganglia encephalopathy on brain magnetic resonance imaging. After ruling out Bickerstaff syndrome, a diagnosis of drug-induced Wernicke’s encephalopathy was confirmed. Chronic disulfiram use may lead to the development of polyneuropathy and Wernicke’s encephalopathy, requiring immediate discontinuation of therapy and vitamin supplementation.
